Job Title: RN - Case Manager (CM)

Job Summary:

The RN - Case Manager (CM) is responsible for coordinating and managing patient care through a comprehensive and patient-centered approach. This role involves assessing, planning, implementing, and evaluating care, ensuring that the patient’s health needs are met, and facilitating communication between healthcare providers and patients.

Key Responsibilities:

Patient Care Coordination: Manage a caseload of patients, ensuring appropriate and timely care plans are developed and followed. Coordinate with healthcare providers (doctors, specialists, therapists) to ensure that the patient’s needs are met effectively.
Assessment and Evaluation: Conduct thorough assessments of patients’ health conditions, both physical and emotional, to determine care needs. Review patient histories, treatment plans, and other relevant data to make informed decisions about care.
Care Plan Development: Develop individualized care plans based on assessments, considering the patient's health goals, preferences, and resources. Adjust care plans as necessary based on changes in patient condition or progress.
Patient Education: Provide education to patients and their families about medical conditions, treatments, and resources available. Empower patients to take an active role in managing their own care.
Advocacy and Support: Serve as a patient advocate, ensuring that the patient’s voice is heard in the healthcare process. Address any concerns or barriers patients may face in accessing care or following prescribed treatments.
Documentation and Reporting: Maintain accurate and up-to-date records of patient care activities. Ensure that all case management activities comply with healthcare regulations, laws, and guidelines.
